Short-Term Disability (SDI) Evaluation
Short-term disability insurance replaces a portion of your income when a medical condition prevents you from working. We provide the clinical evaluation and documentation your insurer requires — entirely via telehealth.
What it is
Short-term disability (STD or SDI) insurance provides partial income replacement — typically 60–80% of your salary — when you are temporarily unable to work due to a non-work-related illness, injury, or pregnancy. Benefits generally begin after a short elimination period (commonly 7–14 days) and last up to 3–6 months depending on your policy. Most insurers require a completed attending physician statement (APS) or equivalent medical certification from a licensed provider.
Why you may need it
Without a completed medical certification from a licensed clinician, your short-term disability claim will be denied. Many primary care providers have long wait times or do not prioritize disability paperwork. Our clinicians specialize in disability evaluations and can complete the required documentation after a thorough telehealth assessment of your condition.

State disability insurance (SDI) programs
Several states operate their own mandatory short-term disability programs, including California (CA SDI), New Jersey (NJ TDI), New York (NY DBL), Rhode Island, Hawaii, and Washington. If you are in one of these states, your employer may use the state program in addition to or instead of private insurance. We can complete documentation for both state and private disability programs.
What your insurer will ask for
Most short-term disability insurers require an Attending Physician Statement (APS) that includes your diagnosis (ICD-10 code), the date your condition began, your functional limitations, expected duration of disability, and the treatment plan. Our clinicians are experienced in completing these forms accurately and thoroughly to support your claim.
