Comprehensive Long-Term Disability Evaluation — Precision Medical Docs

Disability Evaluation

Comprehensive Long-Term Disability Evaluation

Long-term disability insurance replaces a significant portion of your income when a serious medical condition prevents you from working for an extended period. We provide the thorough clinical evaluation and detailed documentation your insurer requires — entirely via telehealth.

What it is

Long-term disability (LTD) insurance provides income replacement — typically 50–70% of your pre-disability earnings — when a medical condition prevents you from working for an extended period, often beyond 90–180 days. Unlike short-term disability, LTD benefits can last for years or until retirement age depending on your policy. Most insurers require a comprehensive attending physician statement (APS) that documents your diagnosis, functional limitations, treatment history, and prognosis. Our clinicians conduct a thorough evaluation specifically designed to meet the documentation standards required by LTD carriers. The $399 evaluation fee includes: a review of your medical records; an in-depth assessment of your medical condition, symptoms, and treatment history; an assessment of occupational demands and functional limitations; and completion of one standard LTD/Attending Physician Statement when clinically supported.

Why you may need it

Long-term disability claims are scrutinized more heavily than short-term claims because the financial stakes are higher for insurers. A vague or incomplete attending physician statement is one of the most common reasons LTD claims are delayed or denied. Our clinicians specialize in disability evaluations and understand exactly what LTD carriers look for — detailed functional capacity assessments, clear diagnostic support, and thorough documentation of how your condition limits your ability to work.

What is included in the $399 evaluation

The comprehensive long-term disability evaluation includes: a review of your medical records; an in-depth assessment of your medical condition, symptoms, and treatment history; an assessment of occupational demands and functional limitations; and completion of one standard LTD/Attending Physician Statement when clinically supported. Documentation is provided only when clinically appropriate following an individualized evaluation.

Own-occupation vs. any-occupation definitions

LTD policies use different definitions of disability. "Own-occupation" policies pay benefits if you cannot perform the duties of your specific job. "Any-occupation" policies require that you be unable to perform any job for which you are reasonably qualified. Our clinicians document your functional limitations in a way that addresses both definitions, giving your claim the strongest possible foundation.

What your LTD insurer will ask for

Long-term disability insurers typically require an Attending Physician Statement (APS) that includes your diagnosis with ICD-10 codes, the date your disability began, a detailed description of your functional limitations (physical and/or cognitive), your current treatment plan and response to treatment, your prognosis, and an assessment of your ability to perform work-related activities. Our clinicians are experienced in completing these forms with the level of detail LTD carriers require.

ERISA and employer-sponsored LTD plans

Most employer-sponsored LTD plans are governed by ERISA (Employee Retirement Income Security Act), which establishes specific procedural requirements for claims and appeals. If your claim is denied, you have the right to appeal. Strong initial documentation from a qualified clinician is critical — it forms the administrative record that any appeal or legal challenge will be based on.

Ongoing care & fee transparency

Patients seeking long-term disability documentation are required to maintain an ongoing relationship with our practice and complete follow-up evaluations regularly — at minimum every 1 to 3 months, depending on the nature and severity of your condition. The frequency of visits is determined by your provider based on your individual clinical needs. Each follow-up evaluation is a separate appointment and incurs additional charges. Consistent, documented ongoing care is essential to support the continuity of your long-term disability claim.

Additional forms, supplemental questionnaires, updated certifications, insurer requests, extensive narrative reports, or documentation requiring additional record review are subject to an additional fee based on complexity. Any additional charges will be disclosed to you before the additional documentation is completed.
