FMLA + SDI Bundle
Many employees need both FMLA job protection and short-term disability income replacement at the same time. Our bundle covers both evaluations in a single telehealth visit at a discounted rate.
What it is
The FMLA + SDI Bundle combines two of our most commonly requested services: FMLA certification (required by your employer to approve job-protected leave under the Family and Medical Leave Act) and a short-term disability evaluation (required by your insurer to approve income replacement benefits). These two processes are often needed simultaneously — when a serious health condition prevents you from working, you typically need FMLA to protect your job and short-term disability to replace your income. Completing both in a single telehealth visit saves time and money compared to booking them separately ($200 + $250 = $450 separately vs. $325 bundled).
Why you may need it
When a serious medical condition forces you to stop working, you are often dealing with two separate bureaucratic processes at the same time: your employer's FMLA paperwork and your insurance company's disability claim forms. Both require documentation from a licensed healthcare provider. Coordinating these separately can be time-consuming and stressful. Our bundle streamlines the process — one appointment, one clinician, both sets of documentation completed and delivered together.

Fast, accurate documentation — 100% telehealth
In a single telehealth visit, our licensed clinicians evaluate your condition and complete both the FMLA certification forms (WH-380-E or WH-380-F for the Department of Labor) and the attending physician statement or insurer-specific forms required for your short-term disability claim. Both sets of documentation are delivered digitally so you can submit them to your employer and insurer without delay.

How FMLA and SDI work together
FMLA and short-term disability serve different purposes but are often used simultaneously. FMLA provides job protection — your employer must hold your position (or an equivalent one) while you are on leave. Short-term disability provides income replacement — your insurer pays a portion of your salary while you cannot work. FMLA leave and short-term disability leave typically run concurrently, meaning the same period of leave counts toward both. (Source: U.S. Department of Labor, dol.gov/agencies/whd/fmla)
What forms we complete
For FMLA, we complete the WH-380-E (Employee's Serious Health Condition) or WH-380-F (Family Member's Serious Health Condition) certification form, as required by the Department of Labor. For short-term disability, we complete your insurer's Attending Physician Statement (APS) or equivalent form. If your insurer uses a proprietary form, please have it available at the time of your appointment.
