FMLA Certification
The Family and Medical Leave Act entitles eligible employees to up to 12 weeks of unpaid, job-protected leave per year. We complete your medical certification paperwork quickly and accurately — 100% via telehealth.
What it is
The FMLA (Family and Medical Leave Act of 1993) is a federal law administered by the U.S. Department of Labor. It requires covered employers to provide eligible employees up to 12 weeks of unpaid, job-protected leave per year for specified family and medical reasons. To use FMLA leave for a serious health condition, your employer will typically require a completed WH-380-E or WH-380-F certification form signed by a licensed healthcare provider.
Why you may need it
Employers are legally permitted to require medical certification before approving FMLA leave. Without a properly completed certification form, your leave request may be denied or your job protection may be at risk. Many employees struggle to get their provider to complete paperwork quickly — our clinicians specialize in this documentation and can complete it efficiently after a telehealth evaluation.

Intermittent FMLA leave
FMLA leave can be taken intermittently — in separate blocks of time or by reducing your normal weekly or daily work schedule — when medically necessary. This is common for chronic conditions that cause flare-ups. Our clinicians can certify intermittent leave needs as well.
