Before Your Appointment: What You Need to Do
To help your visit go smoothly, please complete the following steps at least 24 hours before your scheduled appointment.
Complete Your Intake Documents
Log in to the patient portal and fill out all required intake forms, including your medical history, insurance information, and consent forms.
Please submit these no later than 24 hours before your appointment time.
Upload Required Documents
Please upload the following through the patient portal:
- A clear photo or scan of your driver's license (or other valid government-issued photo ID)
- Any employer forms you were asked to complete or bring (e.g., work status forms, disability paperwork, or return-to-work documentation)
- Any past or current medical records that will support your request for a medical leave
Join Your Video Appointment on Time
Have your device (computer, tablet, or smartphone) charged and ready with a stable internet connection.
- Test your camera and microphone in advance
- Click the video appointment link provided in your confirmation email or portal a few minutes before your scheduled time
Please join promptly, as late arrivals affect your time with the provider and those arriving more than 5 minutes late will be required to reschedule.
